I think Intuitive Surgical is brilliant at what it does. But that doesn't mean I think every hospital should buy one.
I'm a procurement manager for a mid-sized hospital network. I've overseen our capital equipment budget—about $4.2 million annually—for the past eight years. I've signed off on robotic systems from three different vendors, tracked every service contract, and documented every cost overrun in our system. I've been the person sitting across the table from Intuitive Surgical's sales reps, asking the uncomfortable questions nobody else in the room wants to ask.
So when someone asks me about Intuitive Surgical operations inc, they don't get a sales pitch. They get what I've learned—the hard way—about the real cost of owning a da Vinci system.
The view I held until 2021: "Big systems, big benefits"
Everything I'd read about robotic surgery said the same thing: better outcomes, shorter recovery times, fewer complications. And I don't doubt those benefits—for certain procedures. But in 2022, when my hospital was evaluating whether to upgrade from the da Vinci Xi to the then-new da Vinci 5, I decided to do something different. I ran a total cost of ownership analysis.
I tracked every line item: capital cost, installation, training, instruments, service contracts, downtime, and even the electricity cost of running the system for 12 hours a day. Here's what I found.
1. The instruments are the real expense
This isn't a secret. Everyone knows da Vinci instruments are single-use or limited-use. But knowing and feeling are different. When I totaled our instrument costs across 220 robotic procedures in 2023, the number was $680,000. That's roughly $3,090 per surgery. And some instruments—like the EndoWrist graspers—are designed for 10 uses before they auto-lock. Miss the window? That's a new instrument at full price.
If you're doing 50 procedures a year, the math works differently than if you're doing 500. Volume matters. A lot.
2. Training is non-negotiable (and not cheap)
We spent $120,000 on training in 2023. That included sending three surgeons to Intuitive's training center in California, plus the cost of their time away from the OR. And that's before the proctoring fees for the first 10 procedures each surgeon performed on the system. The conventional wisdom is "training is included." My experience suggests otherwise. The basic training is included. The advanced, surgeon-specific training? That's where the real costs appear.
I've never fully understood why the pricing structure for training is so opaque—my best guess is that it's because each hospital has different needs. But I'd love to see a standardized pricing sheet. If someone has insight, I'd love to hear it.
3. The service contract is a hidden multiplier
Intuitive's service contracts are expensive. I'm not going to give you a specific number because it varies by system and region—but I'll say this: in 2024, our annual service contract was 12% of the system's purchase price. That's not unusual for capital equipment. But it's a recurring cost that many people don't factor into their initial budget.
I assumed we'd negotiate a better rate after the first year. Didn't verify. Turned out the contract had an auto-renewal clause with a 4% annual escalator. That mistake cost us $28,000 over three years.
So when does Intuitive Surgical actually make sense?
I recommend the da Vinci system for hospitals that can commit to a high volume of robotic-assisted procedures. Specifically:
- Hospitals performing 200+ robotic procedures annually
- Specialties like urology, gynecology, and colorectal surgery where clear benefits exist
- Programs with a dedicated robotics team (surgeon, nurse coordinator, technician)
If your hospital is doing 50 procedures a year, the per-procedure cost becomes absurd. You'd be better off investing in high-quality laparoscopic equipment and training your surgeons on advanced laparoscopy. The da Vinci is a tool—a fantastic one—but it's not a magic wand.
And what about the medical imaging system? The Ion platform (Intuitive's robotic bronchoscopy system) is a different story. It's designed for lung biopsies, and its value proposition is more straightforward: better access to peripheral lung nodules. But similar cost logic applies. If you're not doing enough lung biopsies to justify the capital outlay, it's not a good investment. I'd say the same about any laparoscope or endoscope system we've evaluated.
Addressing the elephant in the room: "What about competitors?"
I'm not going to name specific competitors—that's not how I do business. But I will say this: the medical device market is not static. New entrants are offering lower capital costs, which is tempting. But price isn't everything. When I compared costs across three vendors over six months, one vendor's initial quote was 40% lower than Intuitive's. But their per-procedure instrument cost was 60% higher. The total cost of ownership? Nearly identical over three years.
The lesson: don't be seduced by a lower price tag. Calculate the total cost to your specific practice.
My honest take
Intuitive Surgical is not for everyone. If your hospital is doing low-volume robotic surgery, or if you're in a specialty without strong evidence for robotic benefit (some general surgeries, for instance), the cost is hard to justify. Being honest about that does not weaken my recommendation for high-volume programs—it strengthens it.
And since someone will ask: no, I don't know anything about dental loupes. That's a completely different medical device category. But I suspect the same principle applies: buy the tool that matches your procedure volume and your team's skill level. It's not about the brand. It's about fit.
